Please help with my nem.

Gaping mouth, that’s not good. Hopefully it resolves by tonight!! A lot fewer filaments visible though so that’s awesome!
He just pooped! Gently blew the stuff off with a baster and scooped it up out of the water

from what I can tell it closed back up already

Looks neither happy nor healthy to me. Perhaps better than before, but the mesenterial filaments still extruding from various spots around the oral disc is not good.

Still not sure what the cause is or how to fix it. Just keeping water in check and letting him have some alone time
 
It has to be a deterioration of the oral disc in those areas. I can think of no other explanation for how insides are showing on the outside. They typically get pushed out of a gaping mouth, but that doesn't seem to be the case here.

At first that is what happened. Then what you see now. As I said in previous post yesterday there was less, today there seems to be a bit more.
I guess right now we will be having good days and bad.

I mean, even without mesenterial filaments, the tentacles are very shortened and stubby, which is also a sign the nem is not healthy/happy. I'm not surprised at all a nem in this condition is also going through periods of mouth gaping. That pooping was more likely expelling of zooxanthellae.
 
I see @OrionN here. Should OP consider cipro treament?

Looks like poop lol. Brown fluffy poop.
 
The anemone actually look pretty good to me. The mesentery filaments are sign that the oral plate is compromised. There are holes in it and the filaments protrude from below. However, the anemone is not deflated. It will heal if you provide it with good, stable water condition. Need to also provide it with adequate light. This is a BTA, if you give it 1/2 a chance, it will recover. I don't think you should treat it with Cipro.

So I have had him covered from the light. I was thinking leaving him uncover when I come home and maybe covering him up before I go to work? So he doesn’t get blasted when the lights ramp up?
 
He does not need to get blasted, but he need energy to heal. Right now, you should not feed him, but if the zooxanthellae can boost his energy intake, it will help him heal.
 
Do you by chance have a sea hare ?
This looks just like an egg mass from a sea hare and the white secretion would make sense. nem looks pretty good in recent pics.

No sea hair. And it’s definitely his insides coming out! I saw the same thing coming out of his mouth right before I made my first post here.
